Applicants must fill in all appropriate information and supporting statements. All information must be provided for the grant request to be considered by the Foundation. 

This includes:


  • Name of organization and contact person


  • Address, city, state and zip code


  • Phone and fax numbers


  • E-mail and website address


  • Brief organization history and description of existing services 


  • Description of the proposed project or program 


  • Specific dollar amount and purpose for the grant requested from the Foundation and date payment is needed


  • All entities asked to provide financial support to the proposed project or program


  • Project or program budget


  • Plans to support the project or program after the grant period, including a statement showing projected income sources 


  • Plan to evaluate the project or program 


  • List of Board members and their financial participation 


  • Names and qualifications of staff involved with the proposed project or program                       

  • The organization's current operating budget and year-to-date financial statements


  • The most recent external audit and most recent IRS Form 990 return filed    


  • A copy of the latest verification from the Internal Revenue Service of tax-exempt status as an organization described in Section 501(c)(3) of the Internal Revenue Code and describing the organization’s status as a public charity under Section 509(a) of the Internal Revenue Code 


  • Certification by submitting individual 


  • A Supporting Organization Information Request Form
